NP - Hospice - Northcentral Wisconsin & Upper Michigan New

Aspirus Health is seeking an experienced Nurse Practitioner to join our Hospice team.

Hospice care is essential in ensuring patients receive compassionate, comprehensive care that honors the whole individual—addressing physical comfort, emotional well-being, and spiritual needs. Through an interdisciplinary, patient-centered approach, hospice supports both patients and their families during one of life’s most meaningful transitions, helping them experience dignity, comfort, and connection.

OPPORTUNITY:

• Provide care wherever a patient calls home, including private homes, skilled nursing facilities, assisted living facilities, and hospice houses, in person and via tele-health
• Serve patients throughout northern Wisconsin and Michigan’s Upper Peninsula
  • Perform face-to-face visits to certify hospice eligibility
  • Partner with interdisciplinary and community resources—social work, chaplaincy, music, pet, aromatherapy, and massage—to support comprehensive hospice care.
  • Heavy emphasis on symptom management
• Benefits eligible position, approximately 3 days a week (flexible)
• Monday-Friday 8:00a – 4:30p
• Experience in Hospice/Palliative Care is preferred
• Experience as a practicing NP is preferred
• 5+ years RN experience required
• Must be licensed in Wisconsin & Michigan, and able to obtain Minnesota license
